Liability Waiver & Release of Liability

Academeer Inc. dba HomeSchool PlayLab

This Liability Waiver and Release of Liability ("Waiver") is executed by the undersigned parent or legal guardian ("Parent") in favor of Academeer Inc., doing business as HomeSchool PlayLab ("HomeSchool PlayLab"), its officers, directors, employees, volunteers, and agents.

1. Nature of Services

I understand that HomeSchool PlayLab is a boutique homeschool learning center and is NOT a licensed daycare facility or registered school. I acknowledge that each session is 75 minutes in duration.

2. Assumption of Risk

I am aware that participation in educational activities and programs involves certain risks, including but not limited to risks of physical injury, accidents, or illness. I voluntarily assume all risks associated with my child's participation in HomeSchool PlayLab programs at the learning center located at The Grove Church, 825 Sam Davis Rd, Argyle, TX 76226, or any other designated location.

3. Release and Waiver

In consideration for my child being permitted to participate, I hereby release, waive, and forever discharge Academeer Inc. (dba HomeSchool PlayLab) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my child's participation in these programs.

This release includes, without limitation, liability for personal injury, property damage, or wrongful death, whether caused by the negligence of HomeSchool PlayLab or otherwise.

4. Medical Treatment

I hereby release and forever discharge HomeSchool PlayLab from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my child's participation. I understand it is my responsibility to provide notice of any medical conditions or allergies.

5. Indemnification

I agree to indemnify and hold harmless Academeer Inc. from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, brought as a result of my child's involvement at HomeSchool PlayLab.

6. Governing Law

I expressively agree that this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Texas and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Texas.

I HAVE READ THIS WAIVER, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT.
